A comprehensive assessment of an individual’s health, care, and support needs.
This includes:
Review of medical history, conditions and medications
(non-prescribing)
Functional and cognitive assessment
Review of services and support
Identification of risks and gaps in care
You will receive:
A clear written summary (plain language)
Practical recommendations
A structured plan of next steps
Advice for continued risk management and what to expect and/or recommendations for continued support
