Silver Willow supports professionals where care has become complex, fragmented, or difficult to coordinate, and where an individual or family would benefit from independent clinical oversight, risk identification, and a clear point of accountability.
What Silver Willow provides
Independent clinical review - Structured review of the individual’s health, care situation, risks, and current support
Coordination across services - Support to align communication, referrals, providers, and next steps across all services
Risk identification and oversight - Early identification of gaps, concerns, and risks that may otherwise be missed
Support for families and decision-makers - Practical clarity for families managing complex situations, especially where strain, uncertainty or distance is a factor
When a referral may be helpful
Multiple professionals or providers are involved and no single person is holding an overall view
An individual’s condition is deteriorating and risks are increasing
Discharge planning or longer-term care planning is becoming complex
Family members are overwhelmed, conflicted, or living off-island
Communication, continuity, or follow-up across services is unclear
An independent overview would support safer, better coordinated decision-making
This is not general administration. It is independent clinical oversight, coordination, and risk management in complex situations.

All referrals begin with an Initial Clinical Review. From there, support may be provided on an ad hoc or ongoing basis according to the individual’s needs, level of complexity, and agreed scope of involvement.

Initial Clinical Review
Core Service
Clinical Assessment and Information A comprehensive assessment of an individual’s health, care, and support needs.
This includes: Review of medical history, conditions and medications (non-prescribing) Functional and cognitive assessment Review of services and support Identification of risks and gaps in care
They or you will receive: A clear written summary (plain language) Practical recommendations A structured plan of next steps
Advice for continued risk managment and what to expect and/or recommendations for continued support
Additional Co-ordination Support
Enhanced Service
For increased involvement in more complex situations Structured clinical assessment of the individuals health, care situation, current support, and immediate risks, with clear written recommendations and practical next steps
Based on indivudal needs and recommendations: Health Co-ordination
Arranging referrals Coordinating providers and medical professionals Co-ordinating discharge plans and transfers of care
Appointment & Communication
Preparing for appointments Structuring communication plans Amending packages
Clinical Review & Monitoring
Review of changes in condition Reviewing medications (non-prescribing)
System & Funding Support
Guidance or completion of applications and representation Sourcing care providers and negotiating packages

An estimate of required hours will be provided prior to service.
Consistent Case Co-Ordination
Complex Service
Ongoing oversight, coordination, and proactive management.
Ongoing oversight, coordination, and proactive review for individuals requiring continued involvement across complex health and care arrangements
Includes: Regular review of health condition, risks and plans Proactive co-ordination of care Priority Contact and agreed response times
Core Package up to 4 hours* (approx.1 hour per week) Monthly updates Priority contact
Enhanced Package up to 8 hours* (approx.2 hours per week) monthly updates priority contact
Complex Package up to 12 hours* (approx.3 hours per week) Monthly updates Priority Contact
*Additional time is charged per hour and agreed ahead of commencement. Service does not provide emergency response, out of hours availability or continuous on call support
Professional Boundaries
Silver Willow works alongside existing professionals and services and does not replace statutory provision, diagnosis, treatment, emergency care, or out-of-hours response. The service is designed to strengthen coordination, continuity, increase understanding and oversight where complexity or fragmentation is creating risk.
